In most states, Individual & Family Plans (IFP) members can earn a $150 prepaid Visa® card reward1 for a primary care provider (PCP) visit and account-related activities through our Health Perks™ program. Earning their $150 reward is simple – the required tasks include:

1. Visiting their PCP, for any reason

2. Registering for an online account at myuhc.com/exchange

3. Opting in for all types of text alerts

4. Signing up for all paperless communications (must opt-in to all email types)

5. Setting up Autopay to pay their monthly premium
